Name Change

If you are changing your name due to a recent marriage, divorce or just legally changing your name from Smith to Jones, it is critical that you also change your records.

Here are some steps to consider:

  • Notify MMBB of the change so we can update your records.
  • If your name change is as a result of marriage, you can submit either a copy of your marriage certificate (not a marriage license) or send a letter to MMBB requesting a name change on your record.
  • If your name change is the result of a divorce you should send a letter to MMBB requesting a name change on your record.
  • If your name change is for reasons other than marriage or divorce, the request must be provided in writing.
  • Contact your employer to change any of the following that are being revised:
    • Your name
    • Your marital status
    • Your address
    • Your dependents
    • Your emergency contacts
    • Your tax withholding

And don’t forget other records, such as Social Security, your driver’s license, your passport, your car registration, your bank, the Post Office, etc.

  • If someone has named you as a beneficiary, you may want to suggest that they update their beneficiary designations to reflect your name change.
  • MMBB recommends you seek legal assistance for the name change. You may want to speak to your lawyer about creating or changing your will or other legal documents, and to discuss beneficiary designations and trust or guardianship arrangements.
