You are eligible to enroll in Benefits for Life as long as you are compensated by a church or 501
Enrollment
Your employer must formally adopt the MMBB Retirement Plan by completing and submitting the Retirement Plan Adoption Agreement stating, among other things, the employee groups that are eligible for participation and the premium percentage they will contribute on the employees’ behalf.
You will then complete an application and beneficiary form for membership in Benefits for Life. Once the application is received in good order, your employer will receive an invoice for your premium payment. Life insurance and disability coverage begins when we receive the initial month’s premium.
